QUEER DISABILITY CONFERENCE 2002 -- REGISTRATION FORM INSTRUCTIONSPre-registration for the Queer Disability conference must be postmarked no later than May 10, 2002. Mail-in registration will be cut off at 250 people; a very small number of walk-in registrations will be available on-site. Access/accommodation requests are due April 30, 2002. NOTE: The entire conference will be a smoke-free and scent-free space,
in order to ensure accessibility for people with chemical sensitivities
and environmental illnesses. Smoking will be allowed in designated areas
only, away from meeting areas. PART 1 -- CONTACT INFORMATION(Please note that the QD conference organizers use email as our primary mode of communication. Therefore, please give us your most reliable, frequently-checked email address.) PART 2 -- CONFERENCE REGISTRATIONThe registration fee includes the two-day conference, Sunday and Monday lunches, and Sunday evening reception. Please note: We have a commitment to making this financially accessible
and no one will be turned away for lack of funds. But we also need to
raise money to offset the costs of the conference. If you need a scholarship,
please pre-register as early as possible and contribute what you can.
For others, registering and paying early helps us by providing funds in
advance and helps us plan. If you need financial assistance with housing or travel please fill out the Scholarship form. PART 3 -- HOUSING AT SFSUDormitory-style housing is available on the campus of San Francisco State
University. These are simple rooms with one or two single beds. These
rooms do NOT have private bathrooms, but share a bathroom with several
other rooms. Costs for on-campus housing at SFSU are as follows: [NOTE: If you decide not to stay on campus, you can find information about area hotels at http://www.sfsu.edu/prospect/hotels.htm] Please note that the conference begins at 9:00 a.m. on Sunday, June 2,
and ends at 5:00 p.m. on Monday, June 3. Please also note that checkout

meeting areas. PART 5 -- CONFERENCE/MEETING ACCESS ISSUESNOTE: The meeting facilities are wheelchair-accessible; and the conference
area will be a smoke-free and scent-free space, in order to ensure accessibility
for people with chemical sensitivities and environmental illnesses. Smoking
will be allowed in designated areas only, away from meeting areas. PART 6 -- AFFILIATION, IDENTITY, AND PROGRAMMING INTERESTSThis information is ENTIRELY OPTIONAL. Your answers will help us to plan the conference programming, including discussion sessions and caucus opportunities. PART 7 -- CONFERENCE TEE-SHIRTSStylish conference tee-shirts, designed and produced by The Nth Degree, are available by pre-order for $15 apiece, IF ORDERED BEFORE APRIL 30. (Some shirts will be available on-site, for $20 apiece, but we won't be able to guarantee availability or sizes.) If you pre-order, you will receive your shirts at check-in.
